Through this evaluation, we will assess implementation or adaption of evidence-based public health strategies, design/develop project-specific performance measures, and quarterly and annual evaluation reports. As allowed, we will also develop project pre/post-tests, surveys, data collection forms or other tools to evaluate project. We will partner with the program on data collection and analysis activities, and prepare quarterly and annual reports to include recommendations for program improvement and submission to DHHS. |
